To Parents/Guardians:

Your student(s) are in the process of being screened as per NYS Screening and Health Exam Requirements by your school nurse for vision, hearing and scoliosis. If your student’s screening(s) are not within normal limits, you will be notified by phone call and referral letter, otherwise this serves as your notification that your student’s screening was within normal limits. If you have any further questions, please contact your school nurse.
