Instructions for Washington DC trip 2023
For all students going to Washington DC: You must complete the form, Health Information for Overnight School Trips. This form can be downloaded here or picked up in the Health Office. These forms are due back BY Monday, March 27th. If we do not receive them, your student will not be able to attend.
Students requiring medication:

You MUST supply a written doctor’s order for any medication that your child needs for this trip, including over the counter medication. Even if your student already has an order for medication in school, they will need the Overnight Medication Form filled out. (This can be downloaded here or picked up at the health office)

If you need more copies of the Overnight Medication Form, please have your child stop into the health office.

Written doctor’s orders must CLEARLY state that the student may self-administer their medication. *If you feel that your child is not capable of self-administering their medication, the doctor may indicate, “May self-administer with supervision”.

ALL medications require an order including, but not limited to, vitamins, allergy medications, nasal spray, OTC pain medication, melatonin, prescription.

ALL medications must be in their original, unopened containers or pharmacy container in which the medication came in. All prescription medication must be clearly labeled by the pharmacy with the student’s name, name of the medication and medication instructions.

Medications should be provided in the smallest available packaging. Please do not send large quantities of medication!

ALL MEDICAL FORMS, DOCTOR’S ORDERS AND MEDICATIONS MUST BE RECEIVED IN THE HEALTH OFFICE BY MONDAY, MARCH 27th.
Washington DC Trip FAQ’s from the Health Office-
Can’t my child bring Tylenol on the trip in their suitcase/purse? No, all medications must be submitted to the health office with proper documentation by the parent, by March 27,2023.

Must I really have a medication order for even non-prescription medications like Tylenol or Advil? Yes, NYS law requires a written prescription for ALL medications, including non prescription drugs.

What about creams, like Hydrocortisone or Calmine Lotion? These are also considered medication and as such must have written orders for them.

Why must I provide the medications/orders so far ahead of time? With 200 students going on this trip, adequate time must be given to process them and sometimes troubleshoot complicated/ incomplete orders.

My child takes medication that can’t be supplied to the school in advance (ie creams, injectable medication, etc.): Please call the health office to make arrangements.

Where can I get more forms? In the Health Office

How will my child have access to their medications if they cannot carry them? They may see their chaperone who will make them available to them at the time they are needed.

Will other students know what medication my child is taking? Your child’s chaperone will be instructed in keeping confidentiality. They will make their medications available on an individual basis.

I work all day. Can’t my child deliver their own medication(s) to the health office? No, Students may not carry medications to and from school (with the exception of emergency medications)

How can I get the medications returned to me after the trip? You may either pick up medications directly from the chaperone when picking up your child, or you may come to the health office to pick them up after the trip.
ALL MEDICATIONS THAT HAVE NOT BEEN CLAIMED BY THE END OF THE SCHOOL YEAR WILL BE DISCARDED.

Cherrydale Fundraising
Our Cherrydale brochures are expected to arrive during the week of January 30th, and will be available in the main office for pick-up. For more information on Cherrydale Fundraising, please visit their website here.
Applebee's Gift Card Fundraiser
Students can sell Applebee's Gift Cards and 10% of their proceeds will go to their trip. The fundraising form can be found here and all checks are to be made payable to "ACSD". The Applebees Fundraiser will begin immediately and run until February 17, 2023. Students can turn in their order forms and checks at any time in the main office before the deadline. Once all of our gift card orders are in, we will submit to Applebee's and the cards will be delivered during the week of February 27th for distribution.
