Important Reminders

  • -Please keep your child’s emergency contact information up to date each school year. In the event of an emergency or illness we need to be able to reach you or an emergency contact person as quickly as possible. Emergency contacts should be readily available to pick your student up.

    -All emergency medication needs to have current orders on file throughout the entire school year. This is a requirement to attend school, as well as for field trips, sports and after school activities.

    - Students in grade 9 and 11 must have a current physical on file 30 days after school starts. Acceptable physicals should be dated September 2024 or later. Our Medical Director will provide a free health appraisal for anyone unable to provide a private physician physical.

    - 12th grade students must receive a Meningitis ACWY vaccine after the age of 16 years BEFORE starting 12th grade. Fourteen days grace period will be given when school begins, then students will be excluded from attending school until proof of vaccination is received.

    Students need to remain home resting if they have:

    • Vomited in the last 24 hours
    • Had diarrhea in the last 24 hours
    • Had a fever in the last 24 hours (without medication)
    • Started antibiotics (including antibiotic eye drops) in the last 24 hours
    • Have a continuous, uncontrollable cough (if it seems unusual check with medical provider)
    • Have unexplained, suspicious rashes (until seen by medical provider)

    Please notify the health office if your student has been diagnosed with a contagious disease - so we can help keep the Arlington Community safe.  NYS mandated form
